Pharmacy
is an evolving profession that
includes more sophisticated
and comprehensive patient care
activities. Such services
include patient health status
assessments; medication reviews;
formulating/adjusting prescription
treatment plans; patient education
and training;
collaborative drug therapy
management; special packaging;
refill reminders; and other pharmacy
related services.
David
Pharmacy, Inc. would like to
introduce two new services we
offer to our community. These
services will be provided in a
private area by a pharmacist whose
time is devoted to the patient
during this service.
"BROWN
BAG" CONSULTATION
By
appointment only, we offer an in-depth counseling -- a special
time set aside for a
"Brown-Bag"
session.
This
Medication Review is a
face-to-face consultation
conducted between the patient or
caregiver and the
pharmacist. This interaction
allows the pharmacist the ability
to observe signs of the patient's
health problems, such as adverse
reactions to medications.
The pharmacist's observations can
result in early detection of
medication related problems and
thus can reduce emergency
room visits,
hospitalizations, unscheduled
physician visits, and overall drug
costs.
For this meeting,
you will bring a bag filled with
all of the medicines you are
taking, including prescription and
nonprescription drugs, herbs
and/or other alternative remedies,
vitamins, minerals, and any other
supplements.
Make sure you bring with you all
of your medication containers and
all of the dosage forms (pills,
creams, ointments, eyedrops,
sprays, skin patches, etc.) of the
drugs and supplements that you
use.
During the session, you will
meet with one of our pharmacists
(Pam or David) to make sure you are only taking
what you need and are doing so
properly.
This
service is provided for a fee of
$38.00
Some
Medicare Part D insurance plans
will cover the cost of this
consultation depending on the
annual cost and number of drugs
you are taking.

COMPREHENSIVE
MEDICATION REVIEW
This is a more
thorough medication review and
includes additional visits
throughout the year to monitor
medication issues or
problems. A typical patient
might need up to four visits per
year, but additional visits would
be available depending on the
individual patient circumstances.
